Summit Medical offers a patient portal to help patients take control of their health care. With our patient portal, it's easy to update
your contact information, communicate directly with care providers, review detailed summaries of your health records, and access
the results of recent lab tests. You can even review your medication list and keep track of your allergies.
Accessing the portal is easy.
Just follow these steps to access your health records or update your information:

Call Summit Medical at (270) 478-5334. Press 3 to speak with the receptionist in patient scheduling.

The receptionist will give you a pre-generated username and password. Write down the information and keep it in a safe place.

Visit www.yourhealthfile.com and click "Activate Your Account." Sign in with the username and password provided by the receptionist.

The first time you log in, you'll be given the option to change the default password. If you decide to change your password, protect your personal information by including special characters, numbers, and a combination of uppercase and lowercase letters.

The YourHealthFile patient portal complies with all patient privacy regulations, ensuring that only you and your healthcare providers
